Forms for In-Person Enrollment 

An original birth certificate for the student, proof of residency within District 113A, parent/guardian identification, and a completed enrollment packet for each new student is required for enrollment.

The first day of student attendance will be Thursday, August 15, 2024. Please register by Wednesday, August 7, 2024 to start the first day. Enrollment is open all year.

Medical, dental and vision forms are not required at the time of your K-8 registration appointment but must be turned in directly to the school before October 15, 2024. Transfer students starting after the beginning of the school year have 30 days to submit all required health forms. Please contact the school nurse with questions.
