PARTICIPATION IN SPORTS/ACTIVITIES PHYSICAL FORM
Procedures for Sports Physicals
1) Sports physicals are valid for ONE year from the date of examination.
2) The ONE year validity period cannot expire at any time during the duration of the sport in which your child is involved.
3) It is recommended that your child undergo the physical after July 1st to cover the entire next school year.
