H1N1 COOK COUNTY SCHOOL VACCINATION PROGRAM

The Cook County H1N1 School Vaccination Program will take place in District 113A schools during regular attendance hours on Wednesday, January 13, 2010. All information and consent forms related to the county’s vaccination program can be found at the link below. On January 4^th, hard copies of the vaccination program packet will be sent home with all Early Childhood through 5^th grade students and sent via e-mail to parents of middle school students.
